The business case for human-centred clinical trials.

Pharmaceutical and biotech companies invest billions of dollars developing new treatments, yet many clinical trials face a costly and persistent challenge: patient dropout.

While discussions often focus on recruitment, retention can be just as critical to study success. Patients who leave a trial early can impact timelines, increase costs, reduce statistical power and delay the delivery of potentially life-changing therapies.

The question is often asked: why do patients drop out?

The answer is usually far more practical than many people realise.

Clinical trial participants are not simply patients. They are parents, caregivers, employees, students and individuals managing complex lives alongside their health conditions. When trial participation becomes difficult to balance with those responsibilities, even highly motivated participants may struggle to remain engaged.

A study may require frequent clinic visits, lengthy travel times, time away from work, childcare arrangements or out-of-pocket expenses. Individually, these challenges may seem manageable. Together, they can become overwhelming.

This is where the industry must rethink how it approaches patient retention.

Too often, clinical research has been designed around operational requirements rather than the realities of patients’ daily lives. A truly patient-centric approach starts by asking a simple question: what barriers might prevent this individual from participating successfully?

The answer will be different for every patient.

For one participant, support may mean transportation to and from clinic visits. For another, it could involve temporary relocation, accommodation arrangements, reimbursement of expenses or bringing certain study visits directly into the home through decentralised services.

Technology has an important role to play in making trials more accessible and efficient. However, technology alone cannot solve every challenge. Patients often need flexibility, empathy and personalised support when unexpected situations arise.

Human connection remains essential. A reimbursement platform may process a standard claim automatically, but it cannot always understand the context behind an unexpected expense or the unique circumstances affecting a patient’s participation. Dedicated patient support teams can provide the problem-solving, reassurance and adaptability that technology alone cannot deliver.

As clinical trials continue to evolve, organisations must remember a fundamental truth: there is no scientific progress without data, and there is no data without people.

The future of clinical research depends on designing studies that fit more naturally into patients’ lives. By reducing participation burdens and treating every patient as an individual rather than applying a one-size-fits-all approach, sponsors can improve retention, strengthen study outcomes and ultimately accelerate the delivery of new therapies to the patients who need them most.

At mdgroup, we believe there is no data without people. That’s why we help sponsors design and deliver patient-centred support programs that reduce barriers to participation, improve retention and ultimately help bring new therapies to patients faster.
